A subsidiary of glass giant Pilkington has been fined £3,000 for failing to report a series of accidents, including one in which a worker had a fingertip sliced off.The incidents came to light when Health and Safety Executive inspector Steve Clarke was looking at the accident book of Nottingham-based Arkwright Glass and Glazing.
