The Health and Safety Executive is investigating the death of a foreman crushed to death on a building site in Wood Green, north London, last week.

John Walsh, 40, from Potters Bar in Hertfordshire, was working on a Mount Anvil job for the London Borough of Harringey, and died from severe head injuries when three excavator buckets fell 3 m on top of him.
