In Brief Rail safety flaw

A THIRD interim report by the Health and Safety Executive into the Paddington rail disaster says it could have been caused by a flaw in safety equipment. The HSE report says that vibration from significant misalignment of the track could have caused safety equipment on the local train involved in the tragedy - which crashed into a high-speed express - to malfunction.
